The following applies to BF Survey Pro version 1.1.3 or greater.

In the menu parameters (component), set "Registered Users Only" to Yes.

If someone tries to access your survey before they have logged in, they will get an error.

Once logged in, you can now access survey, and Name and Email fields are automatically populated.
